Resident-Directed Skilled Nursing Care
Residents of our skilled nursing neighborhood receive long-term care with access to 24-hour, personalized attention and treatment that aligns with their priorities. We create tailored care plans that honor each resident’s values, preferences, and goals.
Each person is an important and cherished part of the community. Rather than focusing on one’s diagnosis, we prioritize whole-person well-being that encompasses physical health, mental wellness, quality of life, and social connection.
We strive to foster a community of support and care between residents, family members, and team members. Family members are always welcome and encouraged to participate in regular care meetings with their loved one and their care partners, and attend community events and activities.
Award-Winning Skilled Nursing Team
Someren Glen’s experienced, award-winning skilled nursing team is comprised of Registered Nurses (RNs), Certified Nursing Assistants (CNAs), Licensed Practical Nurses (LPNs), Physical Therapists, Occupational Therapists, and a Speech Language Pathologist who work together to provide comprehensive, multi-disciplinary care and treatment.
Our skilled nursing neighborhood received the 2024 Customer Experience Award from Pinnacle Quality Insight in the categories of Dignity and Respect, Professional Therapy Services, and Safety and Security. We are honored to be awarded in these categories, as they reflect our commitment to quality care, enriching programs, and a resident-driven culture.
Enriching Care and Programming
In alignment with our mission, we strive to fill each day with meaningful and personalized experiences to empower residents to live well. We believe life enrichment is a whole community responsibility, and approach it from a partnership perspective. Rather than “doing for” residents, we partner with residents to create unique, community-based programming that prioritizes their voices and preferences.
Our Rhythms Life Enrichment philosophy honors each person as a unique individual with their own history, qualities, and interests. This philosophy is based on five pillars of well-being: social, intellectual, physical, purposeful, and spiritual. We celebrate residents’ autonomy and choice by developing activities, classes, and events alongside residents.
We also incorporate the The Eden Alternative approach into community life, building partnerships between older adults and care partners that are focused on continued growth.
Dedicated Skilled Nursing Memory Support Neighborhood
For older adults living with Alzheimer’s or other dementias who also require 24/7 skilled nursing support, our Evergreen Skilled Nursing Memory Support neighborhood expertly integrates care for both physical needs and dementia support.
RHYTHMS DEMENTIA SERVICES
Our holistic approach to memory care – Rhythms Dementia Services – focuses on adapting to each person’s natural rhythm of life, providing customized care to empower residents to safely and successfully live a meaningful life.

The memory support community was carefully designed to offer residents the opportunity to enjoy each day while maximizing safety. Features include:

The staff in the Evergreen memory care wing are some of the most kind, patient, compassionate and attentive service hearted people the world has to offer. This is the 8th destination for my grandmother since May 30. Between hospitals, assisted living, rehabs- not one of them has been what Evergreen is. She’s finally getting the care she needs.
