Someren Glen offers residents friendship, freedom, and opportunity. At our senior living community in Centennial, we’re always on the lookout for new ways to help residents and their families. Since 2021, our relationships with Redwood Health Partners and Perennial Advantage, our Medicare Advantage plan built just for our residents, have provided each older adult access to a new approach of transformative medical services.

Perennial Advantage Plans

Unlike traditional Medicare Advantage plans, Perennial offers continual enrollment due to the special nature of each plan.

• Perennial Advantage Freedom (HMO) – available to residents who have Medicare A & B, and who are living in a Residential/Independent Living or Assisted Living neighborhood. Learn more about the 2025 Freedom Plan.
• Perennial Advantage Strive (HMO I-SNP) – for residents who have Medicare A & B and Medicaid, and who are living in a Skilled Nursing neighborhood or living in an Assisted Living neighborhood and need assistance with two or more activities of daily living. Learn more about the 2025 Strive Plan.
• Perennial Advantage Premier (HMO I-SNP) – for residents who have Medicare A & B, and who are living in a Skilled Nursing neighborhood or living in an Assisted Living neighborhood and need assistance with two or more activities of daily living. Learn more about the 2025 Premier Plan.

Perennial is a customized Medicare Advantage Plan partially owned by our parent-company, Christian Living Communities, for the sole purpose of serving residents’ specific needs. Their mission is to provide better outcomes, increased satisfaction, and lower health costs to our residents. Redwood Health Partners gives Someren Glen residents the convenience of having expert medical care right down the hall or in the privacy of their apartment or suite. A dedicated nurse practitioner hosts regular in-community clinic hours. From regular checkups to managing symptoms of an illness, residents have peace of mind knowing that expert medical support is always within the community. Our dedicated medical concierge is available to coordinate medical visits, lab results, and preventive care.
